Consumer Directed Service Facilitation Services
Medicaid home and community based care services are available to beneficiaries of all ages with many different types of physical and mental disabilities and chronic illnesses. Community-based care also includes a variety of alternative services that can be provided to targeted elderly and chronically ill individuals who are eligible for institutional placement under Medicaid rules. These presentations reflect training sessions held recently for applicable providers.
