Virginia MedicaidImportant COVID-19 Vaccine Update: Request Your Vaccination Record Online
You can use the Virginia Department of Health’s COVID-19 Vaccine Request Portal to easily look up your COVID-19 vaccine record for free.
You only need to enter a few pieces of information to access your records including:
- Name
- Birth date
- Zip code
The portal will provide you with your vaccine history and a QR code you can share with health care providers or others who need your vaccine records. You can also print your records or save them to your computer. The portal is free to use.
The portal includes only COVID-19 vaccinations entered in the Virginia Immunization Information System (VIIS). It may not include vaccinations administered outside Virginia or by federal government agencies such as the Department of Defense or the Department of Veterans Affairs.
